(BMJ)—A 69-yo woman had a 1-yr hx of painless, brown discoloration on her left arm that developed w/in 1 day of receiving an IV infusion of a drug. Within minutes of commencing the infusion, a small induration appeared around the cannula site in the left antecubital fossa. The pigmentation remained unchanged over the course of the year. Which drug was infused 1y prior?
daunorubicin
aminophylline
iron polymaltose
phenytoin
doxycycline
You are correct. Hemosiderin pigmentation is an aesthetically disfiguring consequence of iron leakage into subcutaneous tissues. Unfortunately, only partially successful tx has been reported. On this basis, IV sites should be carefully monitored for signs of extravasation, both during and after infusion.
