(BMJ)—A 79-yo woman w/ diabetes presented w/ trismus and progressive submandibular pain x4 days. Exam: tender, red, indurated submandibular area. CT confirmed the dx. What is it?
Ludwig angina
Viral submandibular adenitis
Tetanus
Bartonella henselae infection
Salivary gland tumor
You are correct. CT revealed a well-demarcated abscess w/ surrounding cellulitis in the submandibular space, also known as Ludwig angina. Ludwig angina typically develops in diabetic pts and can be life threatening, leading to asphyxia, w/ mortality rates near 50% w/o immediate tx. Airway maintenance is crucial, and drainage and abx are indicated. This pt immediately underwent I&D in the OR. Wound cx grew Streptococcus intermedius, and abx were continued. The pt was discharged home w/o complications.
