Then type a starting dose in the top FROM input box. To convert the dose, choose the new medication
in the lower right selector. The converted dose will appear immediately in the TO box.
Notes
- This calculator offers very approximate equipotent dose conversions based on published medical literature and does not account for individual patient variables. Initiating a conservative, individualized dose and titrating to clinical response is essential to safe and effective opioid conversion.
- Direct conversion calculations are not applicable for opioids not included in this calculator. Search "Opioid Equivalency Tools" in the epocrates search bar for additional opioid conversion info.
