(BMJ)—A 55-yo woman w/ well-controlled type 2 DM (and taking metformin) presented w/ recurrent, asymptomatic lesions on her legs. No hx of trauma/bites/other sx. Exam: round, fluid-filled blister on L leg, w/ scars from prior lesions. No neuropathy. Labs: WNL. What is the dx?
Bullous pemphigoid
Bullous diabeticorum
Necrobiosis lipoidica
Diabetic dermopathy
Epidermolysis bullosa acquisita
You are correct. Based on hx and exam, a clinical dx of bullous diabeticorum (diabetic bulla) was made. Bullous diabeticorum is a rare, spontaneous, noninflammatory, asymptomatic, blistering condition usually found in long-standing DM pts. This pt was counselled, and no active intervention was performed. The lesions healed in due course, w/o any complications.
