Adult Dosing .

Dosage forms:  ER TAB: 25 mg, 50 mg, 100 mg, 200 mg

HTN

[50-200 mg PO qd]

Start: 25-100 mg PO qd, may incr. dose qwk; Max: 400 mg/day; Info: tab may be cut in half, but do not crush/chew; taper dose over 1-2wk to D/C; use same total daily dose if switching from immediate-release form

angina

[100-400 mg PO qd]

Start: 100 mg PO qd, may incr. dose qwk; Max: 400 mg/day; Info: tab may be cut in half, but do not crush/chew; taper dose over 1-2wk to D/C; use same total daily dose if switching from immediate-release form

heart failure

[200 mg PO qd]

Start: 25 mg PO qd if NYHA class II or 12.5 mg PO qd if NYHA class III-IV, may double dose q2wk; Max: 200 mg/day; Info: for NYHA Class II-IV heart failure pts w/ reduced ejection fraction; tab may be cut in half, but do not crush/chew; taper dose over 1-2wk to D/C

atrial fibrillation/flutter (off-label)

[50-400 mg/day PO divided qd-bid]
