Adult Dosing .

Dosage forms:  TAB: 10 mg, 20 mg; ER TAB: 30 mg, 60 mg, 120 mg

angina prophylaxis

[immediate-release form]

Dose: 20 mg PO bid; Max: 40 mg/day; Info: space bid doses 7h apart; consider 5 mg PO bid start dose, then incr. to 10-20 mg bid w/in 2-3 days in small stature pts

[extended-release form]

Dose: 30-120 mg ER PO qam; Start: 30-60 mg ER PO qam, may incr. dose after several days; Max: 240 mg/day ER; Info: ER tab may be cut in half, but do not crush/chew

renal dosing

[no adjustment]

renal impairment: no adjustment

HD: no adjustment, on dialysis days admin. after dialysis; no supplement; PD: no adjustment; no supplement

hepatic dosing
