Appropriate Use
weigh risk/benefit; use ketoconazole tablets only when other effective antifungal tx not avail or tolerated
Hepatotoxicity
serious cases, incl. fatal or ones requiring liver transplant, have occurred w/ oral use, even in pts w/o hepatic dz risk factors; inform pts of risk and monitor closely
QT Prolongation and Drug Interactions
contraindicated w/ dofetilide, quinidine, pimozide, cisapride, methadone, disopyramide, dronedarone, or ranolazine due to QT prolongation risk; ketoconazole may incr. drug levels and prolong QT interval, resulting in life-threatening ventricular dysrhythmias such as torsades de pointes
Adult Dosing .
Dosage forms: TAB: 200 mg
infections, fungal
- [200-400 mg PO qd]
- Info: for blastomycosis, coccidioidomycosis, histoplasmosis, chromomycosis, or paracoccidioidomycosis; dose, duration vary w/ infection type, severity; give w/ food
prostate CA, castration-resistant (off-label)
- [400 mg PO q8h]
- Info: use as monotherapy or w/ hydrocortisone; not 1st line tx; give w/ food
Cushing syndrome (off-label)
- [400-1600 mg/day PO divided bid-qid]
- Start: 200-600 mg/day PO divided bid-tid; Info: adjust dose based on clinical response and urinary free cortisol levels or serum cortisol levels
renal dosing
