[individualize dose SC/IV]

Start: 1.25 ng/kg/min SC/IV, may incr. by 1.25 ng/kg/min SC/IV qwk x4wk, then may incr. by 2.5 ng/kg/min SC/IV qwk; Info: if initial dose not tolerated, decr. dose to 0.625 ng/kg/min SC/IV; start treprostinil at 10% of epoprostenol dose when transitioning tx, see pkg insert for full titration; consider SC/IV treprostinil for pts temporarily unable to take PO treprostinil; SC/IV dose (ng/kg/min) = 139 x PO total daily dose (mg) divided by wt (kg); taper dose gradually to D/C
