Adult Dosing .

Dosage forms:  PATCH: 4.6 mg per 24h, 9.5 mg per 24h, 13.3 mg per 24h

Alzheimer dz, mild-moderate dementia

[apply 1 patch qd]

Start: apply 4.6 mg patch qd x4wk, then incr. to 9.5 mg patch qd x4wk, then may incr. to 13.3 mg patch qd; Max: 13.3 mg/24h; Info: start 9.5 mg patch if converting from 6-12 mg/day PO dose; retitrate from 4.6 mg patch if tx interrupted >3 days; do not cut patch
