Adult Dosing .

Dosage forms:  INJ

infections, bacterial

Max: 9 g/day; Info: may incr. to 1.5 g IM/IV q6h for life-threatening infections; dose, duration varies w/ infection type, severity

pneumonia, community-acquired

Info: refer to IDSA guidelines; may D/C after 5 days if afebrile x48-72h and no signs of complications

infection prophylaxis, surgical

Start: w/in 60min preop; Info: may repeat dose q4h intraop

renal dosing

CrCl 10-20: 750 mg q12h; CrCl <10: 750 mg q24h

HD: 750 mg q24h, on dialysis days admin. after dialysis; give 750 mg supplement after dialysis if next maint. dose not due right after dialysis; PD: 750 mg q24h; no supplement
