Cardiovascular Death
pts w/ established cardiovascular dz receiving febuxostat had incr. cardiovascular death rate vs. allopurinol in a cardiovascular outcomes study; weigh risk/benefit when deciding to start or continue pts on febuxostat; should only be used in pts w/ inadequate response to maximally titrated allopurinol dose, allopurinol intolerance, or if allopurinol is not advisable

Dosage forms: TAB: 40 mg, 80 mg
gout prophylaxis

- Start: 40 mg PO qd, incr. to 80 mg PO qd if serum uric acid >6 mg/dL after 2wk; Info: give w/ gout flare prophylaxis such as NSAID or colchicine during tx initiation; consider prophylactic low-dose aspirin tx in pts w/ cardiovascular dz hx; see pkg insert for toxicity-related dose adjustments
