Adult Dosing .

hyperparathyroidism, secondary

Start: 30 mcg PO qhs; Info: for CKD stage 4 or 5 pts w/ severe or progressive hyperparathyroidism; routine use not recommended in pts w/ CKD stage 3, refer to KDIGO guidelines; adjust dose by 30 mcg/day 3mo after start or dose change to target serum iPTH in desired range, vitamin D 30-100 ng/mL, Ca in normal range, PO4 <5.5 mg/dL; D/C if iPTH consistently low, vitamin D consistently >100 ng/mL, or Ca consistently high; may restart at lower dose when resolved
