[metastatic castration-resistant dz]

Dose: 1000 mg PO qd; Info: use w/ prednisone 5 mg PO bid; use w/ GnRH analog in pts w/o bilateral orchiectomy; give at least 1h before or 2h after meals; do not cut/crush/chew tab

[metastatic castration-sensitive, high-risk dz]

Dose: 1000 mg PO qd; Info: use w/ prednisone 5 mg PO qd; use w/ GnRH analog in pts w/o bilateral orchiectomy; give at least 1h before or 2h after meals; do not cut/crush/chew tab

[metastatic castration-sensitive, high-volume dz (off-label)]

Dose: 1000 mg PO qd; Info: for pts w/ de novo metastatic dz w/ at least 4 bone metastases or any visceral dz; use w/ prednisone 5 mg PO qd and docetaxel; use w/ GnRH analog in pts w/o bilateral orchiectomy; give at least 1h before or 2h after meals; do not cut/crush/chew tab
