C. difficile Associated Diarrhea
mild diarrhea to fatal colitis possible on tx or for >2mo after D/C lincomycin; consider dx if diarrhea presentation after abx admin; abx alter colon flora, leading to C. difficile overgrowth; C. difficile produces toxins A and B which contribute to C. difficile associated diarrhea; hypertoxin producing strains cause incr. morbidity and mortality since these infections can be refractory to abx tx and may require colectomy; pt may need fluid, electrolyte, and protein suppl. along w/ abx for C. difficile; surgical eval. as needed; reserve lincomycin for serious infection where less toxic abx inappropriate; avoid use for nonbacterial infection incl. most URIs
