Start: 267 mg PO tid x7 days, then 534 mg PO tid x7 days, then 801 mg PO tid; Max: 2403 mg/day; Info: see pkg insert for toxicity-related dose adjustments; retitrate from 267 mg PO tid if tx interrupted for >14 days; give w/ food
