ACR publishes first lupus guideline update in over two decades

The latest guideline from the American College of Rheumatology provides evidence-based and expert recommendations for treating and managing non-renal systemic lupus erythematosus (SLE). Management of lupus nephritis is addressed separately.
Key recommendations:
- Hydroxychloroquine for all patients unless contraindicated
- Limit glucocorticoid use to the shortest effective duration
- Introduce conventional and/or biologic immunosuppressive therapy early to achieve remission or low disease activity
- Emphasize shared decision-making; recommendations shouldn't restrict access to therapies
Clinical impact: These recommendations aim to reduce SLE-related morbidity and mortality while minimizing medication toxicity and supporting individualized treatment plans for pediatric and adult patients.
