Elevated BP while lying down may predict cardiovascular risk

Supine BP could be a valuable predictor of cardiovascular risk, potentially aiding in the early identification of high-risk patients. Incorporating supine BP measurements into routine clinical practice may enhance risk stratification and guide more effective preventive strategies.
Study details: This prospective cohort study utilized supine and seated BP data from the Atherosclerosis Risk in Communities (ARIC) study to investigate CV risk factors in middle-aged adults from four US communities. The study included 11,369 middle-aged adults with a follow-up period of 27 years to monitor CV disease outcomes.
Results: Supine hypertension without seated hypertension was significantly associated with increased hazard ratios (HRs) as follows:
- Incident coronary heart disease (CHD) and fatal CHD: 1.60 and 2.18
- Heart failure: 1.83
- Stroke: 1.86
- All-cause mortality: 1.43
These associations were consistent regardless of seated hypertension status or hypertension medication use.
