(BMJ)—A 66-yo man who had a liver transplant for alcoholic cirrhosis 3mo prior presented w/ painless masses on his neck, shoulders, upper arms, and upper legs. Exam: multiple nontender soft masses, giving a pseudo-athletic appearance. Lipid profile normal. MRI confirmed the dx. What is it?

You are correct. Madelung dz, also called Launois-Bensaude syndrome, is a rare benign symmetric lipomatosis. The cause is unknown, but alcoholism or liver dysfunction are seen in up to 90% of pts. MRI confirmed diffuse lobulated and symmetric lipomatosis. Madelung dz is treated w/ surgery when needed for functional or cosmetic reasons.
